THE MERCK CO-PAY ASSISTANCE PROGRAM

The Merck Co-pay Assistance Program may offer assistance to eligible, privately insured patients who need help affording the out-of-pocket costs for

KEYTRUDA

or

KEYTRUDA QLEX.

  • Once enrolled, eligible, privately insured patients pay the first $25 of their co-pay per administration
  • Maximum Co-pay Assistance Program benefit per patient, per Eligibility Period is $25,000

Co-pay assistance from the Merck Co-pay Assistance Program is not insurance. Restrictions apply. See Terms and Conditions.

CO-PAY ASSISTANCE MAY BE AVAILABLE FOR PATIENTS WHO

  • Are residents of the United States (including Puerto Rico)
  • Have private health insurance that provides coverage for KEYTRUDA or KEYTRUDA QLEX under a medical benefit plan or a pharmacy benefit plan
  • Have been prescribed KEYTRUDA or KEYTRUDA QLEX for an FDA-approved indication
  • Meet all other Terms and Conditions of the program

The Merck Co-pay Assistance Program is not valid for
patients covered under a Government Program, as that term
is defined in the Terms and Conditions. The Merck Co-pay
Assistance Program is not valid for uninsured patients.

What if I am not eligible for the Merck Co-pay Assistance Program?

You may be able to get help from an independent co-pay assistance foundation. A representative can give you information about these independent foundations.

Each independent foundation has its own eligibility requirements and application process.

THE MERCK PATIENT ASSISTANCE PROGRAM

The Merck Patient Assistance Program provides certain Merck medicines free of charge to eligible patients.

The Merck Access Program representatives can refer you to the Merck Patient Assistance Program.

WHO IS THE PROGRAM FOR?

If you do not have insurance or your insurance does not cover the product you have been prescribed, you may be eligible for free products from the Merck Patient Assistance Program, if you meet certain financial and medical criteria.
